- Parent’s Names
- Parent’s Contact Info
- Son’s Uniform Sizes
- Son’s Choice of Uniform Number (We cannot guarantee your son will receive the number they choose but, we will try to accommodate requests. If there are multiple requests for same number, selection will be determined by seniority)
